Due to the ongoing nature of the COVID-19 pandemic, please review and agree to the following:
• By attending this event, regardless of my COVID-19 vaccination status, I agree to participate in and abide by all safety protocols implemented on site. Including but not limited to: wearing masks in public spaces, practicing social distancing, providing proof of negative COVID-19 test at attendee expense, providing proof of prior vaccination, temperature checking, completing health check questionnaires, etc.
• I agree NOT to attend the event in person if I am experiencing or exhibiting any COVID-19 related symptoms as outlined by the Center for Disease Control and Prevention (CDC), such as fever, dry cough or shortness of breath. For a list of symptoms from the CDC, click here.
• I agree NOT to attend the event if I have become recently exposed to a person with a positive and confirmed case of COVID-19 or if I have experienced any symptoms associated with COVID-19 within the past 14 days.
• By accepting an invitation to the Business Networking Group Event, I expressly agree to adhere to any and all COVID-19 protocols implemented by Jewish Federation of Somerset, Hunterdon & Warren Counties, NJ (JFedSHAW), any facilities or venues at which the event is to be held, or any third-party organizations providing services, recreation or entertainment at the event. Agreement by you to comply with applicable COVID-19 protocols is a condition of your invitation to this event.
• I acknowledge the contagious and evolving nature of COVID-19 and voluntarily assume the risk that I may be exposed to the virus and become infected as a result of my attendance at the Business Networking Group event. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any expense, liability, illness, injury, disability or death related to testing positive for the virus during or after the event. I hereby release, covenant not to sue, discharge, and hold harmless JFedSHAW from all claims arising out of COVID-19. I understand and agree that this release of liability includes any claims against JFedSHAW, as well as event personnel or representatives, whether a COVID-19 infection occurrence exists before, during or after attendance and participation at the event.
• I, on behalf of myself, my heirs, and my personal representatives, do hereby agree to release, covenant not to sue, discharge, hold harmless and defend JFedSHAW, its officers, directors, agents, employees, event personnel and. representatives, from all claims arising out of COVID-19, I understand and agree that this includes any claims, whether a COVID-19 infection occurrence exists before, during or after attendance and participation at the event.
• By attending JFedSHAW’s Business Networking Group event on September 14, 2021, you voluntarily assume all risks related to exposure to COVID-19.

I hereby grant the permission, without reservation, to Jewish Federation of Somerset, Hunterdon & Warren Counties to take and to use photographs and/or sound/image recordings of me, to describe and to use the same for promotion of good will, public education, and/or fundraising and other related activities of the Federation, and I waive any right to inspect or approve the photograph(s) or finished version(s) of works, including website, incorporating the photograph(s).

I release Federation, its officers, trustees, agents, employees, independent contractors, licensees, and assignees (including photographers) from all claims that I may have or might have, for any cause of action arising out of taking and/or use of the photographs and/or any sound/image recordings, and/or description of the same, be it blurring, distortion, alteration, optical illusion, or use of composite form whether intentional or otherwise, that may occur or be produced in taking of photographs, or any processing toward the completion of the finished product, unless it can be shown that they and the publication thereof were maliciously caused, produced, and published solely for the purpose of subjecting me to conspicuous ridicule, scandal, reproach, scorn, and indignity.

I recognize that Federation owns the copyright (or may apply for copyright) of these photographs and other works and creations, and I hereby waive any claims I may have based on any usage of the photographs or works derived therefrom in any form, whether it be printed, projected, televised, or transmitted via the web, or/and at any time, be it in the present or in the future, including, but not limited to claims for either invasions of privacy or libel.

I am of full age and competent to agree to this release. I agree that this release shall be binding on legal representatives, my heirs, assigns, and me. I have read this release and I fully understand its contents.
